Health Coach-Client Agreement

1. Scope of Services

1.1 The Coach agrees to provide health coaching services to the Client. These services include personalized guidance, goal setting, progress evaluation, and recommendations related to nutrition, lifestyle, and general wellness practices.

1.2 The Coach’s services do not constitute medical advice, diagnosis, or treatment. The Client acknowledges that the Coach is not a licensed medical practitioner, psychologist, or dietitian and that health coaching is not a substitute for professional medical care.

1.1 The Coach agrees to provide health coaching services to the Client. These services are designed to offer personalized support and guidance for individuals with ambitious health and wellness goals. Services and obligations by the Coach and Client include:

  • Completion of a comprehensive questionnaire by the Client to provide necessary background information for the Coach.
  • An initial 45-minute Zoom session between the Coach and the Client to explore the Client’s goals, challenges, and priorities.
  • Bi-weekly 45-minute Zoom sessions between the Coach and the Client to discuss progress, perform nutrition analysis, and address topics such as energy levels, sleep, stress, motivation, and activity levels.
  • Written follow-ups after each session, summarizing discussions and providing advice and support for the upcoming period.
  • Weekly check-ins to assess the Client’s nutrient profile and provide tailored written feedback with actionable advice for the week ahead.
  • Messaging support for the Client between check-ins for guidance or assistance as needed.
  • Access to additional Nutrivial resources and information to support the Client in achieving their goals.
  • The option to schedule additional Zoom calls with the Coach for an extra fee.

1.2 The goals of the coaching relationship between the Client and the Coach will be formulated by the Client in collaboration with the Coach during the initial session. These objectives will be documented by the Coach and provided to the Client following the session. Any significant changes to the objectives will be recorded by the Coach and communicated to the Client.

1.3 The Coach’s services do not constitute medical advice, diagnosis, or treatment. The Client acknowledges that the Coach is not a licensed medical practitioner, psychologist, or dietitian and that health coaching is not a substitute for professional medical care.

2. Scope and Duration of the Relationship

2.1 This Agreement establishes a coaching relationship for a minimum duration of three (3) months, starting on [Start Date] and concluding on [End Date].

2.2 After the initial 3-month period, the Client may continue the coaching relationship on a month-to-month basis or terminate the Agreement with written notice provided at least three (3) business days before the start of the next billing cycle.

2.3 Either party may terminate this Agreement at any time for breach of its terms, with written notice. No refunds will be issued for unused services if the Client chooses to terminate the Agreement before the end of a paid period.

3. Privacy and Confidentiality

3.1 The Coach will maintain strict confidentiality regarding all personal information shared by the Client. This includes any health, lifestyle, and personal data discussed during the coaching relationship.

3.2 Information shared during the coaching sessions will not be disclosed to any third parties without the Client’s prior written consent, except as required by law.

3.3 The Client acknowledges and agrees that electronic communications (e.g., email, video calls) used in the coaching process, while secure to the best of the Coach’s ability, may carry inherent privacy risks.

4. Fees and Invoicing

4.1 The Client agrees to pay a fee of EUR 300/150/120 per month/session for health coaching services.

4.2 Fees are to be paid in advance at the beginning of each monthly period. Payment must be made by acceptable payment methods e.g., credit card, bank transfer.

4.3 Invoices will be issued prior to the next billing period, and payment must be received by the due date specified on the invoice.

4.4 Late payments may result in suspension of services until payment is received. Consistent failure to pay on time may result in termination of this Agreement.

5. Client Responsibilities

5.1 The Client agrees to actively participate in the coaching process, including setting goals, completing any assigned tasks, and providing honest and accurate information about their health and lifestyle.

5.2 The Client understands that the success of the coaching relationship depends on their own commitment and effort.

5.3 The Client acknowledges that results are not guaranteed and that progress depends on various factors, including adherence to agreed-upon strategies and recommendations.

6. Limitations of Liability

6.1 The Client agrees that the Coach shall not be held liable for any damages, injuries, or adverse outcomes arising from the Client’s participation in the coaching program.

6.2 The Client is responsible for seeking medical advice from a licensed healthcare provider before implementing any recommendations provided by the Coach.

7. Entire Agreement

7.1 This Agreement constitutes the entire understanding between the Coach and the Client concerning the health coaching relationship and supersedes all prior agreements, whether written or oral.

7.2 Any amendments to this Agreement must be made in writing and signed by both parties.

8. Governing Law

8.1 This Agreement shall be governed by and construed in accordance with the laws of Switzerland.
